What these terms mean

IDPH's facility lookup lists each survey it closed at the establishment, with the exit date and the survey type, and posts the Statement of Deficiencies (form 2567) when the survey found the rules were not met. Each cited item names the section of the state rules for assisted living and shared housing (77 Ill. Adm. Code Part 295) that was not met.

Survey types are IDPH's own: an Annual Licensure Only Survey is the yearly licensing inspection; a Complaint Original Investigation (COI) looks into a complaint; a Facility Reported Incident (FRI) looks into an incident the establishment reported. 24HR, 7D and 30D give how soon IDPH had to start.

IDPH's rules (77 Ill. Adm. Code 295.1050) define the violation types. “Type 1 violation - an act or omission by the establishment or its staff that causes severe harm or the death of a resident or creates a substantial probability of severe harm to a resident or residents.” “Type 2 violation - an act or omission by the establishment or its staff that causes harm to a resident or creates a substantial probability of harm to a resident or residents.” “Type 3 violation - an act or omission by the establishment or its staff, except by accidental means, that causes a significant negative impact on the delivery of services to the residents of the establishment.” Labels such as General Violation or Repeat Violation are shown as IDPH wrote them on the statement.

Each statement begins with IDPH's own summary of the survey, quoted here; for complaints and incidents it usually says whether the allegation was substantiated and whether a violation was cited. This establishment is licensed for 120 units; larger establishments are surveyed and cited more often, so read counts with size in mind. How Lilisity works. The only scores or rankings Lilisity publishes are the Top Communities list and the Excellence Awards, both built from phone-verified resident and family interviews. Communities can pay Lilisity a flat monthly fee to manage their own listing: photos, prices, contact details, and receiving the inquiries families choose to send them. The fee is the same no matter how many families contact them or move in. Senior Living Alliance, which runs Lilisity, offers the same listing to its member communities as part of their membership; members pay SLA for its resident and family interview program. No community pays Lilisity per inquiry, per referral or per move-in. Payment never changes a testimonial, a rating, or the order of search results. The "Nearby Senior Living Alliance communities" box shows member communities only. How we verify testimonials · How we are paid
